To schedule an appointment, you will be required to provide a credit or debit card to be kept on file. You will be charged the full appointment fee 24 hours prior to your appointment time.

Insurance

Dr. Jones is an “out-of-network” provider and does not accept insurance. After an appointment, you will have access to a “superbill” to submit to your insurance company for reimbursement.

cancellations / no shows

If you are unable to keep your appointment, please reschedule your appointment as soon as possible through the online patient portal or by calling our office. If you cancel or reschedule less than 24 hours before your appointment, or fail to show for your appointment, you will be charged the full fee. There is no charge for rescheduling greater than 24 hours prior to your appointment.

services not offered

Dr. Jones does not offer evaluations for forensic evaluations, court-related testimony, child custody determinations, mandated treatment, disability, worker’s compensation, or emotional support animal letters. Evaluations related to other medical, work, or legal matters are subject to Dr. Jones’ discretion.
